More than a decade ago, Dr Bellaney became the first skin specialist to offer Tumescent Liposuction in the South Island. We use Tumescent Liposuction to improve the shape of the:

Tumescent liposuction
Liposculpture at the Encorė Cosmetic Clinic is carried out using the technique of Tumescent liposuction.
It can be used for fat reduction in the breasts (male or female), legs, arms, buttocks, abdomen, hips, waist and neck and jowls.
Dr Bellaney is a New Zealand Dermatologist who has been a practising Cosmetic Dermatology since 1996. He has been performing micro-liposuction since 2000, and has received training in tumescent liposuction in both Australia and the United States and Europe. He set up the South Island’s first specialist Tumescent Liposuction clinic in Christchurch in 2004. He is one of New Zealand’s most experienced Tumescent Liposuction practitioners.
How does Tumescent liposuction work?
The word ‘tumescent’ means swollen and firm. Dr Bellaney injects diluted anaesthetic and epinephrine (a small blood vessel constrictor) into subcutaneous fat, making the targeted tissue ‘tumescent’.
The tumescent technique provides local anaesthesia of subcutaneous fat, which means the entire liposuction procedure can be performed under local anaesthetic. The process eliminates both the need for general anaesthesia or IV narcotics (intravenous pain killers) and sedatives. Therefore there is usually no post-operative nausea or vomiting that are frequently associated with general anaesthesia. Intravenous fluid replacement is also not necessary.
Because tumescent local anaesthesia lasts a long time, tumescent liposuction is less painful than traditional liposuction under general anaesthesia or IV sedation.
Tumescent vasoconstriction (the constriction of blood vessels) is so good that the liposuction can be performed with virtually no blood loss, as opposed to traditional forms of liposuction where blood loss can be an issue.
Postoperative Healing
Normal healing after tumescent liposuction involves a limited but definite degree of soreness, swelling, bruising, itching and lumpy firmness. Rarely a temporary mild numbness, itching or burning of the skin may persist for 2 to 10 months. Most patients can actually see some improvement of their silhouette within one week after surgery. However, because of the slow resolution of post-surgical swelling, the ultimate results following liposuction usually require 12 to 20 weeks to be achieved.
TriLipoTM is a new non-invasive fat removal technology that offers an alternative to liposuction for small areas of the body or those areas that require only a ‘slight’ improvement. It is literally a machine that is able to reduce fat deposits and improve stretch marks and cellulite without the need for surgery.
The Maximus Tripollar® system at the Encorė Cosmetic Clinic represents the latest development in tripollar radiofrequency and is the first of its kind in New Zealand. We call it TriLipoTM.
TriLipoTM technology is the most advanced solution for body sculpting available. Along with cryolipolysis, it is the only FDA approved technology shown to reduce fat. The triple action system decreases and removes unwanted fat deposits, tightens the skin, reduces cellulite and shrinks stretch marks. As the name implies, it uses a triple action mechanism simultaneously and is the only system in the market that does so.
The ‘no downtime’ procedure has been globally embraced. It is often also used on the body with great success in treating cellulite and circumferential reduction.
How does TriLipoTM work?
Having treatment with our Maximus TriLipoTM is comparable to having an ultrasound – using some gel, a handpiece is moved over the site to be treated. A feeling of warmth typically occurs but it is not painful.
Fat cells are, by nature, very different from other cells in the body, particularly in how they respond to stimuli such as temperature. The TriLipoTM radio-frequency technology delivers energy to the targeted areas of fat. By driving controlled heat deep within the fat cells it subsequently destroys them.
Most clients realise the optimal outcome at the conclusion of the full treatment course – a recommended five treatment sessions. There is usually a noticeable reduction in fine body lines, contouring and mild lifting of the treatment area, and some improvement of the skin tone and texture. Patients benefit from both visible immediate and long-term results. The immediate effects include a smoother tightened appearance and texture.
Since there is no downtime with this procedure, patients can resume their regular activities immediately.
Large breasts can cause chronic pain and discomfort. They can hold you back from living the healthy, active life you desire. The Tumescent Liposuction carried out at the Encorė Cosmetic Clinic is significantly less invasive than traditional surgical breast reduction methods, and can be highly-effective at reducing the size of the female (and male) breast.
This form of liposuction can be used to both reduce the size of the breast and produce breast lift (due to the reduced weight of the breast).
The breasts do not change shape as a result of the procedure but become smaller overall.

At the Encorė Cosmetic Clinic we treat tired eyes with eyelid surgery or autologous fat transfer (or a combination of the two).
Eyelid Surgery or Blepharoplasty is for the treatment of the upper and/or lower eyelids. It targets the ‘bags’ under our eyes.
This is a procedure that involves the removal of any excess eyelid skin. Depending on the desired outcome and clinical assessment, fat and possibly also a small amount of muscle are also sometimes removed. However in some patients, particularly on the lower eyelids it may be best to add fat (see fat transfer below) to some parts of the eyelid rather than remove it.
Fat Transfer
Fat Transfer (correctly termed Autologous Fat Transfer or AFT) is the process by which a patient’s own fat is removed from one area (usually the abdomen, flanks or buttock), processed and placed into facial areas of concern to the patient. This is most commonly the lower eyelid, cheek, eyebrow and temporal areas.
Fat transfer will restore the ‘fullness’ of the treated area that is usual in younger faces.
Is Eyelid Surgery (Blepharoplasty) or Fat Transfer best?
Lower Eyelids
In recent times it has become appreciated that as we all age we tend to lose fat from our face. The best approach to the ageing lower eyelid is now thought by many to be replacement of fat into this area rather than removal of eyelid fat. At the same time as replacing fat (i.e. fat transfer) any excess skin is removed.
Upper Eyelids
The main problem here is usually excess, loose skin. Often the hollow between the eyebrow and upper eyelid will also benefit from fat transfer.
Cheeks and Temporal Areas
With age, these areas also tend to loose fat and ‘hollow-out’. Fat transfer to these areas is also very good at restoring a more youthful appearance to the face.
Dr Bellaney has been performing blepharoplasties for more than a decade. However, now rather than performing only eyelid surgery (blepharoplasty), in many patients he uses a combination of fat transfer and excision of loose skin for the best results.
Autologous Fat Transfer for Face/Hand Rejuvenation
Fat Transfer (correctly termed Autologous Fat Transfer or AFT) is the process by which a patient’s own fat is removed from one area (usually the abdomen, flanks or buttock), processed and placed into the areas of concern to the patient such as the face or hands. It can also be used in the lower eyelid, cheek, eyebrow and temple areas.
Fat transfer will restore the ‘fullness’ of the treated area that is usual in younger faces.
How does Autologous Fat Transfer work?
Fat is removed with a very minor tumescent liposuction procedure. Typically, a small amount of tumescent anaesthetic is placed into the fat around the abdomen or buttocks. Approximately 20-50mls of fat is removed. This process is usually very well tolerated.
Once the fat is extracted it is prepared for transfer and placed into small vials. The area/s of fat loss to the face are marked out and local anaesthetic is placed at a few specific locations on the face. A period of waiting will occur while the anaesthetic numbs the nerves suppling the skin to the treated area. It can take 10-30 minutes to treat each area. It is usual to feel a sense of pushing and pressure during this time but it should not be painful.
Any area of fat loss can be treated, including the cheeks, temples and eyebrow areas.
Autologous Fat Transfer or AFT requires the establishment of blood supply to the transferred fat in order to become permanent. Fortunately in the area below the eye, most of the fat injected remains permanent and so only one to two treatments are generally needed. In other areas often only 20-30% of the fat transferred remains permanent, so four to five procedures may be required for the best results.
How long does the procedure last?
Unfortunately the ageing process and slow loss of our facial fat continues despite surgery! However, the fat replaced by AFT generally takes many years off the appearance of a patients face.
